Referrals & Legal Notices
Referrals can be made via a consultant, medical, dental or health practitioner or by self-referral. The Form dietitian will in the case of self-referral always contact your GP.
Form Dietitians are registered with the Council for Professions Supplementary to Medicine to ensure a high level of professional standards, which protect the patient from receiving the incorrect advice.
Form Dietitians do not give free advice unless seen as part of an NHS consultation.
If you are seeking advice through private medical insurance always check that the costs will be covered before attending an appointment with the dietitan.
