How do I make an appointment?Form dietitians will except a referral from your self, GP, consultant or other health professional. Form dietitians, do not provide free advice, unless agreed, otherwise through private medical insurance companies (this service is currently pending). To make an appointment at the London or Brighton consulting rooms, you can initially make contact by email or telephone and the dietitian will contact you to make an appointment. In all cases the dietitians will contact your GP as formality and if any clarification of medical results or treatment are required. All information is strictly confidential; dieititians are bound by the Health Professional Council code of conduct. What will I need to bring to a consultation?It advisable to come along in comfortable clothing. If you are seeing the dietitian for sports / health related consultation, the dietitians will want to take your body weight, waist measurement and skinfold measurements from your legs, stomach, back and arms If you are taking any supplements or prescribed medication, it is useful to bring a list of these, including the dose amounts. Any relevant medical history will be helpful, for example; diagnosis of diabetes, osteoporosis, history of anaemia etc. If you keep a training log, personal training plan and performance aims, then this is also very useful to bring along to the initial consultation. What will the dietitian want to know?This depends largely on why you are seeing the dietitian. As a guide the dietitian on the first consultation will want to build-up a picture lifestyle/sport/medical history and how this is all bonded together. The dietitian, may ask you to complete a food and exercise/symptom diary. FORM dieititians use a scientific analysis database to analyse your diet. How many times will I need to see the dietitian?Again this depends on why you are seeing the dietitian. If for example you were seeing the dietitian for general healthy eating advice and evaluation of your diet with your lifestyle or perhaps you wanted your calcium intake reviewed in view of osteoporosis then normally one appointment would be suffice, possibly one follow-up. If you are seeing the dietitian as it is fitness performance related then often at least 2 appointments are recommended. If you are seeing the dieititian for weight loss, then often the follow-up is ongoing until your goal is met. What training have FORM dietitians undertaken?All FORM dietitians have completed a 4 year Bsc Hon degree at University, with clinical training within an acute hospital for accreditation with the health professional council as accredited registered dietitian. All Form dietitians are specialist dietitians also working with the NHS with at least 4 years clinical and strategic experience.
